Dual Diagnosis and Justice Case Management
Offered by COTA Health

Case management for dual diagnosis clients who have involvement with the criminal justice system.
Case Managers work with the client and his/her circle of care on a time-limited basis to develop a formal goal plan based on the client’s unique desires and needs; Teach skills related to goals, such as using the TTC, meal preparation, etc.; Link to a variety of community services and develop a support network that may include housing, employment, social/recreation opportunities, education, psychiatric assistance, etc.; and meet with client regularly to work on goals and follow up on progress.
