Person-Directed Planning
Offered by Community Living Toronto

Community Living Toronto offers person-directed planning free of charge for people who are not receiving any other services from our agency. Person-directed plans explore who a person is and what his/her goals are. This includes how s/he participates in the neighbourhood; her/his likes and dislikes, hobbies, interests, talents, strengths, fears, concerns; what s/he want as well as what supports and connections are needed to achieve these goals. A variety of planning tools are used to assist with the development of a written plan which includes an action plan with timelines.
