| Description: | Addus provides individualized programming and support in life skills, vocation and recreation, that enable adults with developmental disabilities to actively participate in their community. Programs provided by Addus include… Day Supports – Are available to participants anywhere from one to five day per week commencing at 8:30am and concluding at 3pm each day. Specific day support activities are chosen based on the personal goals and desired outcomes of each participant. Day support activities may include; learning kitchen skills, cooking, baking, international cooking, arts and crafts, music, fitness activities, swimming, walking, yoga, indoor and outdoor games, learning and using office and telephone skills in a practical setting, learning TTC skills by actually practicing them in the community, spa and healthy living activities, library time, learning internet skills, practicing word skills, learning and practicing pedestrian safety, neighbourhood outings and explorations, learning about and discussing current events, cultural research, engaging in community service activities, book club, learning ‘how to’ skills, learning and practicing independence, and more. Community Connections Programs Include... Social Club – Allows individuals to engage in a variety of leisure activities and provides an opportunity to socialize, establish outside connections and develop valued social roles within the community. Social club takes place every Tuesday and Thursday night from 3pm until 5:30pm. Participant may spend time at the program site playing cards, air hockey, foosball or yoga. Alternately they may decide to do some baking, preparing healthy snacks or do some juicing. Outside of the site participants may choose to go for a neighbourhood walk, visit the library, engage in fitness activities at the close by community centre, go bowling, play bocce ball and more. Saturday Events/Respite – This program allows individuals to explore and develop personal interests by engaging in special events or visiting Toronto attractions from 2pm – 7pm every Saturday. Participants may travel to various areas throughout Toronto and engage in leisure activities such as shopping at the Eaton Centre, Yorkdale Mall and St. Lawrence Market, or instead they may choose to go bowling, skating or enjoy a day of dinner and a movie. Alternately, visits to a number of Toronto attractions and parks may be scheduled to places such as Nathan Phillips Square, Christie Pits, The Science Centre, Ontario Place, CN Tower, Riverdale Farms, Black Creek Pioneer Village and more. Participants also enjoy taking in the sights, sounds and flavours of cultural events, street festivals and Sinfonia concerts. Vocational Program – Uses Personal Outcome Measures as a philosophy and a tool in an effort to support individuals towards independence and self-fulfillment through vocational opportunities. Vocational programs provide each participant with 3 hours weekly of vocation experience with an Addus Job Coach. Currently Addus has a number of vocational partners including Starbucks, Kent Daycare, Good Life Fitness, The Body Shop and more. Individualized Support Program (ISP) – Provides one on one support to individuals in an effort to explore and develop personal goals, interests and social roles. ISP typically occurs in 3 or 4 hour blocks of time, either during program hours, in the evenings or on weekends. ISP allows participants to work on personal goals and interests that may include developing TTC independence, skating, playing pool, swimming and other social and recreational activities. Passport Program – Is a new and innovative program that seeks to develop individualized placements within the community. At this point Addus has been successful in placing Passport participants with such organizations as The Daily Bread Foodbank, Kent Daycare and Starbucks. Additional relationships are currently being developed with animal rescue organizations, Meals on Wheels, seniors homes, Habitat for Humanity, karate clubs, book stores, libraries, bakeries, community centres, Red Cross, Heart and Stroke Foundation, HMV and more. |
