Eligibility
Adults 16 years of age or older; have a developmental disability and a disgnosis of serious mental illness. Dual Diagnosis and Justice clients must have recent involvement with the criminal justice system.

Description
COTA Health is an accredited provider of mental health and community support services. We provide support to adults living with serious mental illness, geriatric mental health conditions, acquired brain injury, developmental disabilities and dual diagnosis. Our services include case management, supportive housing, short-term residential beds, a day program, court and justice related services and hostel outreach for individuals who are homeless or living in shelters. In the near future we will be implementing a recovery-oriented Assertive Community Treatment Team as part of a Mental Health Commission of Canada research demonstration project.

COTA Health was founded in 1973 as Community Occupational Therapy Associates (COTA) to provide homecare therapy services. In July 2009, COTA Health shifted focus to exclusively provide mental health and community support services.

COTA Health’s services are funded by the Central LHIN, the Ministry of Health and Long Term Care, the Ministry of Community and Social Services, the City of Toronto and the Mental Health Commission of Canada.
