Planners from Community Living Toronto use a variety of visual planning tools to engage individuals and those in their personal networks to develop a person-directed plan, including PATH and MAP, as well as custom-designed tools such as Muse and Passport. As of July 2, 2007, requests for person-directed planning should be made through Information and Access Service of Community Living Toronto. |
